Covid-19 vaccine passport for people living in Thailand

 

The Department of Disease Control of the Ministry of Public Health in Thailand can issue international Covid-19 certificates of vaccination through offices indicated to the PDF file below.

Please note that any Covid-19 certificates of vaccination issued in Thailand are not recognized by Luxembourg authorities. Indeed, in Luxembourg, all vaccination certificates follow strictly the European Union (EU) rules and are therefore in their format and content identically to EU vaccination certificates issued by other EU countries.

When you have received your complete COVID-19 vaccination you will be issued a Thailand-specific certificate of COVID-19 vaccination from the place where you are vaccinated. If you would like to travel overseas, you may request a vaccine passport for the COVID-19 vaccination for foreign travel. 

Conditions for COVID-19 Vaccine Passport

  1. You must have completed a course of the COVID-19 vaccination.
  2. You must travel within three months of the issuance of the vaccine passport. 

Required Documents

1. If applying yourself:

  • Original citizenship identification card (if applicable) and a signed copy.
  • Original passport with at least six months of validity and a signed copy.
  • Original Thailand National Certificate of COVID-19 Vaccination that confirms that you received a course of the COVID-19 vaccination, which can be issued from the place you received your vaccine, and a signed copy.
  • Electronic airline ticket that specifies the date of travel within three months of the issuance of the vaccine passport.
  • Fee of 50 Baht per passport.


2. If a representative will be applying for the passport for you,
two sets of documents are required:

Set 1: Documents for the person traveling

  • Original citizenship identification card (if applicable) and a signed copy.
  • Original passport with at least six months of validity and a signed copy.
  • Original Thailand National Certificate of COVID-19 Vaccination that confirms that you received a course of the COVID-19 vaccination, which can be issued from the place you received your vaccine, and a signed copy.
  • Electronic airline ticket that specifies the date of travel within three months of the issuance of the vaccine passport.
  • Fee of 50 Baht per passport.

Place of Issue

Currently there are four organizations that can issue the vaccine passport under the jurisdiction of the Department of Disease Control, the Ministry of Public Health:
